I've still not found an answer to this. I have verified that
these thumbnail images of pages are optional. The Adobe PDF
Reference, 5th edition, v1.6 [1] states:

Is it possible to prevent XEP from generating and including these
thumbnail images?

The only mention of "thumbnail" in the XEP 4.7 Reference for Java
[2] is in section "4.3. PDF Viewer Preferences (PDF, PostScript)"
where the show-thumbnails option is discussed. This option,
though, simply indicates whether a PDF viewer should initially
show the thumbnails pane when displaying the document.

A search [3] of the RenderX support site for "thumbnail" turns up
lots of discussion of numbering of thumbnail pages, but no mention
of enabling or disabling the generation and inclusion of them in a
PDF document.
