Hello Graham,
Regarding 'span' warning cited in your mail: XSL-FO specification
allows fo:block with 'span' property set to 'all' only as a direct
child of fo:flow. Then XEP encounter spanning block nested deeper
in the document it issues a warning. It seems to be a known problem
with Docbook stylesheets: http://lists.oasis-open.org/archives/docbook-apps/200310/msg00242.html
Though I don't know that's the status of this issue for the current
Docbook stylesheets - you may want to ask about it on the docbook-apps
mailing list.
