I have a requirement for a back-of-the-book index
with two-column layout in which the top of a
column should indicate if an index entry is
continued from the previous column.  This needs
to work for primary and secondary entries.
The worst case is like this:
hard drive                   [bottom of one column]
  formatting
    access to utilities
    caching drive info
[column break]
hard drive (continued)       [top of next column]
  formatting (continued)
    fast format
    slow format
  
Is this possible to do with XSL-FO?  I know about markers
in the static content for region-before for page headers.
Can this be done using that mechanism?  If not, any ideas
on how to implement this?
