    I may have mis-attributed the procedure and the parts of the DocBook

    If so, apologies to the author of the code in the DocBook XSL stylesheets
    -- presumably Norm Walsh or Bob Stayton. :-)

    That aside, if anyone's using the RenderX extensions (rx:key and
    rx:page-index in particular) with DocBook as an alternative to these
    approaches, please let me know.
