Can you post an example illustrating the problem?

When I need to test specific features of XSL-FO or a vendor's tool, I
usually modify a sandbox instance in which I can play with the
constructs I need. Can you reproduce the problem in the following?

<?xml version="1.0" encoding="US-ASCII"?><!--raw.fo-->
<root xmlns="http://www.w3.org/1999/XSL/Format"
       font-family="Times" font-size="20pt">

   <layout-master-set>
     <simple-page-master master-name="frame"
