For the border, change your stylesheet to put the border-top on
the fo:table-cells in the table-header rather than on the table
element itself. Then the borders will be repeated when the
header repeats.

The "continued" item is an XEP extension you can implement using
a technique described in their FAQ. Look for
rx:table-omit-initial-header in:
