By default XEP prints all logging messages to STDERR. You can redirect
them using standard means when running XEP from command-line. You can
also create your custom logger as I've already mentioned.

I think that you should look into XEP intermediate format issued by
the formatter core: http://www.renderx.net/Content/support/xep/reference.html#appendix_C
Is already paginated and contains information about IDs in the form of
xep:target elements.

Note also that there is an intrinsic indexes support in XEP provided
via extensions to XSL-FO: http://www.renderx.net/Content/support/xep/reference.html#Indexes
